About the role

The OP Home-Based Physical Therapist provides comprehensive physical therapy services, including assessments, treatment planning, and therapeutic interventions, adhering to professional and ethical standards.

Responsibilities

• Conduct evaluations and treatments for patients of all ages as prescribed by physicians
• Deliver therapies such as muscle strengthening, gait training, and equipment management
• Perform evaluations and document progress
• Develop and adjust treatment plans with clear goals
• Educate patients and caregivers
• Communicate with healthcare providers
• Manage scheduling and transportation
• Maintain departmental policies, equipment, and safety standards
• Monitor insurance authorizations
• Perform additional duties as assigned

Qualifications

• High school diploma or GED
• Doctor of Physical Therapy (post‑2015)
• Licensure as a Physical Therapist
• Valid Texas Driver’s License
• Auto liability insurance
• BLS certification (to be obtained within 90 days)
• Dependable transportation to fulfill job duties
