Home-Based Physical Therapist

Role Overview: The Home-Based Physical Therapist provides comprehensive physical therapy services, including assessments, treatment planning, and therapeutic interventions, adhering to professional and ethical standards. They contribute to program development, quality improvement, and interdisciplinary problem-solving. Key Responsibilities Conduct evaluations and deliver treatments such as muscle strengthening, gait training, ADL training, and device management for patients of all ages.Perform initial and ongoing patient assessments, develop personalized treatment plans with clear goals, and monitor progress.Maintain accurate, timely clinical documentation in compliance with department and payer guidelines.Instruct patients and caregivers on activity routines, ensuring understanding through written and verbal instructions.Communicate regularly with healthcare providers, coordinate patient care, and manage appointment schedules efficiently.Transport portable therapy equipment, uphold department policies, and participate in safety and quality programs.Manage productivity, monitor insurance authorizations, and address scheduling or patient concerns proactively.Perform other duties as assigned to support department operations. Qualifications High School Diploma, GED, or equivalent required; Doctor of Physical Therapy (DPT) degree after 2015.Licensed Physical Therapist and valid Texas Driver’s License required.Certifications in auto liability insurance and Basic Life Support (BLS) within 90 days of hire.Must have reliable transportation for the role.
