Home-Based Physical Therapist

Role Overview: The Home-Based Physical Therapist provides comprehensive physical therapy services, including assessments, treatment planning, and therapeutic interventions, adhering to professional standards and ethical practices. They contribute to program development, quality improvement, and interdisciplinary collaboration. Key Responsibilities Conduct evaluations and administer treatments to patients across all ages, from adolescents to geriatrics.Deliver therapies such as muscle strengthening, stretching, gait training, ADL training, and manage orthotic/prosthetic devices.Perform initial and periodic patient evaluations, documenting progress and adjusting treatment plans with clear goals.Complete accurate and timely clinical documentation in compliance with guidelines.Instruct patients and caregivers on activities to support treatment outside sessions.Coordinate with healthcare providers, providing updates and modifying care plans as needed.Manage appointment scheduling, transportation, and portable therapy equipment efficiently.Maintain department policies, safety standards, and participate in quality programs and departmental development.Monitor insurance coverage and authorizations, communicating concerns to management. Qualifications High School Diploma or GED required; Doctor of Physical Therapy (DPT) degree required after 2015.Licensed Physical Therapist and valid driver’s license essential.Auto liability insurance and BLS certification required within 90 days of hiring. Skills & Experience: No specific experience required; candidates must demonstrate reliability, effective communication, and organizational skills. Individuals must have dependable transportation throughout employment.
