Heartland Bridges Case Manager III (overnights, full time)
Summary Description Of Program
Heartland Bridges houses two programs, BridgesTransitional Living and Bridges Crisis Care.
BridgesTransitional Living is a short-term transitional housing program enriched with supportive services designed to assist people with a diagnosed mental health or co-occurring condition while they transition to stability in the community.
Bridges Crisis Care is a residential stabilization service designed for individuals who need a safe, secure environment for three to five days while stabilizing from a crisis before returning to the community where they live.
Summary Description Of Work
The role of the Case Manager III (Overnight) is to provide facility oversight, assist in duties to ensure the day-to-day operations of the program, and respond to client needs.
Decision making, problem solving, and highly developed interpersonal skills are critical.
Pay & Schedule
Compensation: between $19.74 and $23.54 per hour (wage is determined by total years of relevant experience)
Schedule
Work Hours: four overnights per week (10:00 p.m. to 8:00 a.m.)
Qualifications
- Minimum Qualifications
- Associate degree in human services field (including but not limited to, psychology, social work, mental health counseling, marriage and family therapy, or a related field), Bachelors preferred
- Two years of experience in behavioral or mental health services
- One year of experience working with adults with serious mental illness or substance use preferred
- Valid drivers license and acceptable driving record
Essential Duties And Responsibilities
- Responds to crisis care referral calls, assesses individual by gathering information and provides appropriate and accurate application verdict, maintaining strict confidentiality
- Completes enrollment/intake on eligible Crisis Care clients
- Develops supportive relationship with clients and responds to client needs during shift
- Demonstrates knowledge of current community and HFS programs and resources to meet client needs
- Completes monthly client record reviews
- Conducts client room searches per program protocol
- Orients clients to Bridges protocols and addresses client non-compliance as necessary to create a safe and secure environment for all residents and staff
- Attends All Staff meetings as scheduled
- Able to work both independently, with little supervision, and as a part of a team
- Always displays a courteous and caring attitude to the clientele, volunteers, and visitors of the Agency
- Abides by all specific program and Agency procedures, policies, and requirements
- Cooperates and collaborates with program area staff, volunteers, and other Agency staff
- Creates, maintains, and shares as appropriate a dynamic self-care plan
