Summary Description Of Work
The Heartland Homes program provides facility-based Permanent Supportive Housing in the form of long-term financial assistance and supportive services using a Housing First model to individuals and families experiencing long-term homelessness and those who have a disability. This position works out of two facilities in Council Bluffs, IA, targeting families with high service needs and providing intensive case management services.
Compensation
Compensation: Between $19.74 and $23.54 per hour (wage is determined by total years of relevant experience).
Work Schedule
Work Schedule: 37.5 hours per week (flexible with evening/weekend hours: 5pm-12:30am).
Minimum Qualifications
Bachelor's Degree in Social Work or Human Service field, and two years direct care experience in Human Services preferred; or equivalent
Valid Drivers License and acceptable driving record
Essential Duties And Responsibilities
- Aid the daytime Case Manager with applicant assessments, including accepting, denying, and referring applicants.
- Verify and document the eligibility of applicants and maintain comprehensive service records for all approved clients.
- Assist clients in identifying strengths, needs, and goals, including meeting obligations of tenancy to maintain their home and reducing risk of harm to themselves and others.
- Help clients increase financial security by accessing, maintaining, or improving employment and/or benefits.
- Provide, either directly or by referral to other agencies, planned and crisis response services to meet the needs of the household, including concrete supports, interventions to reduce stress and mitigate trauma histories.
- Teach/coach clients to develop communication and self-advocacy skills.
- Ensure safety and security measures for in-home services are followed.
- Maintain accurate and timely documentation, including service records, case notes, and incident reports.
- Maintain positive working relationships with clients, referral sources, and others encountered in the course of work.
- Provide problem-solving and crisis intervention services to empower clients to develop self-sufficiency.
- Monitor security cameras to ensure clients are adhering to their lease and client/facility safety.
- Regularly inspect and conduct routine walkthroughs of Transitions and Heartland Homes facilities to ensure safety and security measures are in place and operating.
- Coordinate completion of repairs to facilities according to Heartland Family Service policies and procedures.
- Perform routine janitorial tasks in public/common areas, including ensuring regular cleaning of bathrooms, floors, vacuuming, trash removal, and window cleaning.
- Demonstrate knowledge of current community and Heartland Family Service programs and resources to effectively meet client needs.
- Evaluate efficiency and effectiveness of community service providers on an ongoing basis to ensure clients are receiving quality care.
- Able to work independently, with little supervision, and as a part of a team.
- Utilize trauma-informed care and culturally competent principles in providing services to clients.
- Able to establish regularity and dependability in scheduling all appointments and accuracy in reports and statistics.
- Show a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the Agency.
- Absorb all specific program and Agency procedures, policies, and requirements.
- Collaborate with staff, volunteers, landlords, and referral sources.
- Travel is required, including use of personal vehicle if agency vehicle is not available.
- Be dependable and punctual regarding scheduling and attendance.
- Ability to evaluate HFS program services and make recommendations.
- Create, maintain, and share as appropriate a dynamic self-care plan.
- Strive to make connections between the agency and the larger community whenever possible in order to contribute to the agency's ongoing fundraising and friend-raising efforts.
