About the role

Community Solutions is seeking a Case Manager to join our team at the Stein House location in Hartford, CT.

Responsibilities

  • Complete assessments within 3 business days of intake.
  • Facilitate feedback and creation of ISP within 5 business days of intake.
  • Submit monthly progress reports to your supervisor 4 calendar days before due date.
  • Collect and review phone records on a monthly basis to ensure compliance with the SOW.
  • Prepare, plan and facilitate Transitional Skills (Wilmington, WH, HH and Chase) and R&R group-sessions.
  • Complete and submit discharge plan for BOP and DOC client 60 days before scheduled release date.
  • Maintain client files and ensure compliance with Agency policy regarding confidential information.
  • Assist clients with obtaining birth certificate and social security card.
  • Conduct CM sessions at the persons home confinement site.
  • Secure urine specimen and Breathalyzer test at the HC site.
  • Conduct a search of the clients vehicle while at the HC site.
  • Collect and make copies of the clients bank records to ensure it balances with our financial records.
  • Complete incident reports as needed and submit to PD prior to leaving shift.
  • Register for and attend all required trainings and boosters including but not limited to MI, Case Management and Assessments.
  • Conduct weekly audits of all client medications to ensure clients are med compliant.
  • Conduct employment, job search and home confinement spot checks.
  • Conduct room searches, common area searches and perimeter searches.
  • Conduct pat/pocket searches, facility rounds, and house counts as needed.
  • Aid clients in obtaining employment and maintaining home confinement compliance.
  • Model pro-social behaviors to clients and staff.

Qualifications

  • Bachelor's degree in a social science field and one year experience in case management or similar field.
  • Education may be substituted with experience on a year for year basis.
  • Excellent oral, written and interpersonal skills desired.
  • Ability to engage and develop collaborations with community agencies.
  • Previous assessment experience preferred.

Skills

  • Strong communication and interpersonal skills.
  • Proficiency in Microsoft Office.

Schedule

Monday/Wednesday 12:00pm to 8:00pm and Tuesday/Thursday/Friday 9:00am to 5:00pm

CSI understands clients and staff have been impacted by trauma. CSI is becoming a trauma-informed organization by hiring individuals with existing experience in trauma-informed practices as well as supporting ongoing staff development with trauma-informed practices. This will include learning to recognize symptoms of trauma and how to respond, through training starting at orientation and throughout their career at CSI. CSI is committed to systematically evaluating the integration of the trauma-informed values of safety, trustworthiness, choice, collaboration, and empowerment at all levels of the organization's policies, procedures, and structures to guard against the possibility of re-traumatization.
