About the role

As a Case Manager working toward certification, you'll build valuable experience while helping clients overcome challenges, access resources, and make progress toward their personal goals.

Responsibilities

• Provides psychosocial rehabilitative services as described by the Medicaid manual
• Serves as a liaison between clients and other healthcare providers
• Supports clients in following individualized care plans and identifying case management goals
• Completes documentation on services provided within Valley’s standards
• Maintains accurate and timely records of client interactions and progress
• Maintains confidentiality of client information
• Distributes medications as necessary
• Transports clients to appointments as necessary

Requirements

• High School diploma or equivalent
• CPR certification
• Valley de-escalation certification
• Minimum age of 21 for driving positions
• Valid driver’s license for driving positions

Pay

Dependent on certification status Uncertified Case Manager range starts at $17.50 - Certified Case Manager pay is calculated based on years of certified experience
