Program Overview
The Heartland Bridges program houses two programs: Bridges Transitional Living and Bridges Crisis Care. Transitional Living offers short-term housing with supportive services for individuals with diagnosed mental health or co-occurring conditions transitioning to community stability. Crisis Care provides a secure environment for three to five days to stabilize individuals from crises before returning to the community.
Job Summary
The Case Manager III provides intensive case management services to Heartland Bridges clients, focusing on wellness, recovery, and stabilization. This role involves crisis response, intake, service planning, and support services. The position requires strong decision-making, problem-solving, and interpersonal skills.
Compensation and Hours
Compensation ranges from $19.74 to $23.54 per hour, based on years of relevant experience. Work hours are 40 per week, 2:30 p.m. to 10:30 p.m.
Qualifications
Associate degree in human services field (psychology, social work, mental health counseling, marriage and family therapy, or related)
Bachelor's degree preferred
Two years of experience in behavioral or mental health services
One year of experience working with adults with serious mental illness or substance use
Essential Duties and Responsibilities
Respond to crisis care referral calls, assess individuals, and provide accurate verdicts while maintaining confidentiality.
Complete enrollment/intake for eligible clients.
Aid clients in identifying strengths, needs, and goals to develop individualized service plans.
Provide support services, including safety and stabilization plans, and connect clients to providers.
Document client activity, service plan progress, and needs according to established procedures.
Offer problem-solving and crisis intervention services, empowering clients toward self-sufficiency.
Screen for mental health and substance use treatment needs and provide linkages to services.
Assist clients in finding and retaining suitable housing.
Develop supportive relationships with clients and respond to their needs.
Maintain and support the facility's upkeep.
Conduct client room safety checks and searches according to program protocol.
Accurately record client outcomes in designated databases.
Provide psychoeducation and Life Skills Inventory coaching sessions.
Demonstrate knowledge of community and HFS programs and resources.
Utilize trauma-informed care, person-centered, and culturally competent principles.
Evaluate the efficiency and effectiveness of community service providers.
Orient clients to Bridges protocols and address non-compliance.
Perform essential functions at the agency's physical work site.
Perform other program-related duties as assigned.
