Responsibilities
- Evaluate and treat patients
- Coordinate services such as specialty referrals and hospitalizations
- Participate in team huddles
- Ensure timely documentation
- Follow clinical policies and support quality improvement initiatives
- Engage in community relationships
- Work closely with the care team to enhance patient outcomes
- Spend meaningful time with patients
- Participate in on-call programs
Requirements
- Board-certified Nurse Practitioner or Physician Assistant
- Active, unrestricted license
- Strong communication and interpersonal skills
- Adaptability to a value-based care environment
- Basic computer proficiency and EMR experience
- Credentialed to prescribe medications
Qualifications
- 2-5 years of relevant experience preferred
- Preferred DEA license
- Medicare/Medicaid numbers
- Experience managing Medicare Advantage panels
- Knowledge of Medicare guidelines and HEDIS indicators
