Responsibilities
- Evaluating and treating patients following care standards
- Coordinating services such as referrals, hospital and SNF liaison, and home health
- Leading and participating in team-based care activities
- Ensuring timely medical documentation and compliance with policies
- Engaging in quality improvement and growth initiatives
- Participating in on-call programs and community outreach
Qualifications
- Board-certified Nurse Practitioner or Physician Assistant with active license
- 2–5 years of relevant experience preferred
- Excellent communication and interpersonal skills
- Ability to adapt to value-based care models
- Basic computer proficiency and certification for prescribing medications
- Willingness to participate in TB screening and maintain compliance with licensing standards
Preferred Skills
- Active DEA, Medicare, and Medicaid provider numbers
- Experience managing Medicare Advantage panels and understanding of Medicare guidelines
- Knowledge of HEDIS quality indicators
