About the Role

This position involves providing in-home clinical care to high-risk patients within an interdisciplinary team. The role emphasizes comprehensive assessments, managing acute and chronic conditions, and conducting advanced care planning and patient education.

Responsibilities

  • Performing detailed in-home evaluations, including medical history, vital signs, and screenings
  • Managing acute medical episodes and chronic condition exacerbations
  • Ensuring thorough documentation and collaborating with care team members
  • Engaging in advanced care planning and providing patient and caregiver counseling

Qualifications

  • Active certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care for Nurse Practitioners
  • Current NCCPA certification and state licensure for Physician Assistants
  • Unrestricted licensure, valid driver’s license, reliable transportation, and prescriptive authority
  • Ability to navigate varied environments and perform physical duties

Preferred Skills and Experience

  • 1+ years clinical experience in geriatric, home health, or long-term care settings
  • Experience with complex behavioral, social, or functional patient needs
  • Knowledge of geriatrics, chronic illness, and end-of-life discussions
  • Proficiency with electronic medical records and technology
