About the role
Provide in-home care to high-risk patients as part of an interdisciplinary team, focusing on holistic health management.
Responsibilities
- Conduct comprehensive in-home assessments, including medical history, vital signs, and screenings.
- Manage acute medical issues and exacerbations of chronic conditions through testing and interventions.
- Coordinate care during short-term rehabilitation episodes.
- Document accurately, including ICD 10 coding, and collaborate with team members.
- Engage in advanced care planning, counseling, and patient education.
Requirements
- Active certification (ANCC/AANP for NPs or NCCPA for PAs) and unrestricted state licensure.
- Valid driver’s license and reliable transportation.
- DEA licensure or ability to obtain it.
- Ability to operate in varied environments and perform physical duties as needed.
Preferred Qualifications
- 1+ years clinical experience.
- Geriatric or home health background.
- Proficiency with electronic medical records.
