Healthcare Provider - Full-Time

About the Role

Provide in-home clinical care to high-risk patients as part of an interdisciplinary team, focusing on holistic health needs. Conduct comprehensive assessments, manage acute and chronic conditions, and facilitate advanced care planning. Collaborate with team members, document thoroughly, and educate patients and caregivers.

Responsibilities

• Perform medical history reviews, physical exams, screenings, and point-of-care testing
• Manage patients during acute episodes and short-term rehabilitation
• Ensure accurate documentation, including ICD-10 coding
• Communicate effectively within the care team
• Engage in advanced care planning and patient education

Requirements

• Active, unrestricted licensure and certification (ANCC, AANP, or NCCPA)
• Valid driver’s license and reliable transportation
• Current DEA license or ability to obtain
• Ability to navigate diverse environments and perform physical duties

Preferred Qualifications

• Clinical experience in geriatric, home health, or long-term care settings
• Proficiency with electronic medical records
• Experience with complex patient needs
