About the role

Provide in-home clinical care to high-risk patients as part of an interdisciplinary team, focusing on prevention, chronic condition management, and care coordination.

Responsibilities

• Conduct comprehensive assessments
• Manage acute and sub-acute episodes
• Document thoroughly
• Engage in advanced care planning and patient education
• Daily travel within assigned areas, with no night, weekend, or holiday shifts

Qualifications

• Active licensure (NP or PA) with relevant certifications and ability to obtain DEA prescriptive authority
• Reliable transportation and active driver's license
• Ability to navigate varied environments and perform physical duties as needed

Skills

• Preferred: 1+ years of clinical experience, especially in geriatric, home health, or chronic illness management
• Preferred: proficiency with electronic health records
