The role of Public Health Nurse III - Adult Health/Diabetes Coordinator involves providing comprehensive health care services to adult populations, focusing on diabetes management and public health initiatives.

Responsibilities

• Develop and implement diabetes prevention and management programs
• Provide individualized health education and counseling to patients
• Collaborate with community partners to enhance public health outcomes
• Conduct health assessments and develop care plans
• Monitor patient progress and adjust care plans as necessary

Requirements

• Bachelor’s degree in Nursing or related field
• Current licensure as a Registered Nurse (RN)
• Experience in public health nursing or diabetes management
• Strong interpersonal and communication skills
• Ability to work independently and as part of a team
