About the role
The role of Public Health Nurse III - Adult Health/Diabetes Coordinator involves providing comprehensive health care services to adult populations, focusing on diabetes management and public health initiatives.
Responsibilities
- Develop and implement diabetes prevention and management programs
- Provide individualized health education and counseling to patients
- Collaborate with community partners to enhance public health outcomes
- Conduct health assessments and develop care plans
- Monitor patient progress and adjust care plans as necessary
Requirements
- Bachelor’s degree in Nursing or related field
- Current licensure as a Registered Nurse (RN)
- Experience in public health nursing or diabetes management
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
Qualifications
- Knowledge of diabetes management protocols
- Understanding of public health principles and practices
