Health Mentor - Family and Community Medicine

About the role

The Health Mentor will provide recovery-oriented peer support and skills training services, teaching classes and providing mentoring sessions to assist community members with serious mental illnesses, substance use disorders, and/or general mental health conditions. The role focuses on increasing individuals' knowledge, skills, and motivation to improve their health, reach their recovery goals, live independently, and integrate into their communities.

The Health Mentor will also participate in community outreach recruitment events and activities.

Responsibilities

• Provide recovery-oriented peer support and skills training services.
• Teach classes and conduct mentoring sessions for community members with serious mental illnesses, substance use disorders, and/or general mental health conditions.
• Assist individuals in increasing their knowledge, skills, and motivation to improve health and achieve recovery goals.
• Support individuals in living independently and integrating into their communities.
• Participate in community outreach recruitment events and activities.
