Health Mentor - Family and Community Medicine

About the role

The Health Mentor will provide recovery-oriented peer support and skills training services, teaching classes and providing mentoring sessions to assist community members with serious mental illnesses, substance use disorders, and/or general mental health conditions. The role focuses on increasing individuals' knowledge, skills, and motivation to improve their health, reach their recovery goals, live independently, and integrate into their communities.

The Health Mentor will also participate in community outreach and recruitment events and activities.

Responsibilities

• Provide recovery-oriented peer support and skills training services.
• Teach classes and conduct mentoring sessions for community members with mental health conditions, substance use disorders, or serious mental illnesses.
• Assist individuals in improving their health, achieving recovery goals, and living independently.
• Support community integration for program participants.
• Participate in community outreach and recruitment events.
