Family Medicine Physician- Community Health

About the role

The Family Medicine position at Optimus Health Care is available at our Stratford clinic. As a leading provider of primary health care services in Fairfield County, we are seeking a dedicated physician to join our team-based model of care.

Responsibilities

• Provide comprehensive medical care to patients in a family medicine setting
• Collaborate with other healthcare providers to develop and implement treatment plans
• Conduct routine check-ups, diagnose illnesses, and prescribe medications as needed
• Offer preventive care services including screenings and vaccinations
• Participate in interdisciplinary team meetings to discuss patient cases
• Adhere to HIPAA guidelines and maintain patient confidentiality

Requirements

• Medical degree from an accredited Medical School
• Completion of a Residency from an accredited institution specializing in Family Medicine
• Current Connecticut Licensure or application in process
• Board Certification or Eligible to sit for exam

Qualifications

• Professional, positive attitude, vision, understanding of customer service principles, trustworthiness, and excellent interpersonal skills
• Experience in a multi-specialty medical home setting
• Strong communication and problem-solving skills
• Ability to work independently and as part of a team
