Health Mentor
About the role
Southwest Behavioral and Health Services is seeking a dedicated and empathetic Health Mentor to join our dynamic In Shape team at our Lake Havasu City Outpatient Clinic. The ideal candidate will enjoy coordinating with people, solving problems, being organized, and communicating effectively.
Responsibilities
- Assists InSHAPE members in developing and implementing their personal wellness program.
- Provides support, education, and motivation related to physical fitness, nutrition, and wellness.
- Encourages by example and provides individual and group activity instruction.
- Works independently in community settings with individual consumers or leading activities.
- Collects data on participant progress.
- Supports the activation of the Recovery Philosophy, including a person-centered approach based on self-determined, self-directed, and self-managed care.
- Facilitates quarterly celebration/recognition events for participants.
- Works with clients, volunteers, and community partners to increase client independence.
Qualifications
- Minimum requirement: 21 years of age.
- Personal Trainer Certification or willingness to obtain certification (SBH covers certification).
- High school diploma or GED and 4 years of full-time health experience, or related associate’s degree and 3 years of full-time health experience, or bachelor's degree in Health Sciences or Social Work with 1 year of behavioral health experience (preferred).
- An equivalent combination of education and experience directly related to behavioral health totaling 5 years is acceptable.
- Valid Arizona driver’s license and pass motor vehicle screening for insurance purposes.
- Fingerprint clearance issued by the Arizona Department of Public Safety or eligibility to receive within 8 weeks of hire.
- Maintain personal auto insurance per SBH guidelines.
