Full Time Hospice Case Manager

As a Hospice RN with AdvisaCare, you’ll provide one-on-one care in the comfort of home, supporting patients and families with dignity and heart. Spend real time where it matters most, delivering gentle, personal, and deeply meaningful care without the chaos of hospital halls. No on-call requirements, as we have a dedicated team for after-hours and weekends.

Responsibilities

• Initial and ongoing assessment of patient’s physical, functional, psychosocial, and environmental needs
• Implement and revise the individualized plan of care as needed
• Ensure compliance with AdvisaCare Hospice Policies and Procedures and Medicare requirements
• Consult with and educate patients and families on disease processes, self-care techniques, end-of-life care, and ethical concerns
• Supervise paraprofessionals providing services to patients
• Prepare clinical and progress notes and participate in interdisciplinary group (IDG) conferences
• Participate in the Hospice performance improvement program
• Maintain a high level of customer satisfaction with clients, families, and caregivers
• Communicate information using current processes and technology within the organization
• Conduct supervisory visits to observe and evaluate assigned team members
• Provide feedback during regular interdisciplinary group meetings for the development, review, and revision of patient/family plans of care
• Participate in field visits daily and attend weekly office meetings as indicated

Requirements

• Current Michigan RN license in good standing
• Minimum 1 year of hospice experience
• Empathetic approach to patients and caregivers
• Ability to adapt to patient’s status and needs on a full-time basis
