Full Time Hospice Case Manager

About the role

As a Hospice RN with AdvisaCare, you’ll provide one-on-one care in the comfort of patients’ homes, supporting patients and families with dignity and compassion. This role focuses on gentle, personal, and meaningful care without the chaos of hospital settings. You’ll spend real time where it matters most, honoring each moment with presence and heart.

No on-call requirements—we have a dedicated team for after-hours and weekends.

Responsibilities

• Conduct initial and ongoing assessments of patient’s physical, functional, psychosocial, and environmental needs.
• Implement and revise individualized plans of care as needed.
• Ensure compliance with AdvisaCare Hospice Policies and Procedures and Medicare requirements.
• Consult with and educate patients and families on disease processes, self-care techniques, end-of-life care, and ethical concerns.
• Supervise paraprofessionals providing services to patients.
• Prepare clinical and progress notes and participate in interdisciplinary group (IDG) conferences.
• Participate in the Hospice performance improvement program.
• Maintain high levels of customer satisfaction with clients, families, and caregivers.
• Communicate information using current processes and technology within the organization.
• Conduct supervisory visits to observe and evaluate assigned team members’ performance.
• Provide feedback during regular interdisciplinary group meetings for the development, review, and revision of patient/family plans of care.
• Participate in daily field visits and weekly office meetings as indicated.

Requirements

• Current Michigan RN license in good standing.
• Minimum of 1 year of hospice experience.
• Empathetic approach to patients and caregivers.
• Ability to adapt to patients’ changing status and needs on a full-time basis.
