Full Time Hospice Case Manager

As a Hospice RN at AdvisaCare, you’ll provide one-on-one care in the comfort of home, support patients and families with dignity and heart, and spend real time where it matters most. Care without the chaos of hospital halls, the way it’s meant to be—gentle, personal, and deeply meaningful. No on-call requirements; a dedicated team covers after-hours and weekends.

Responsibilities

• Initial and ongoing assessment of patient’s physical, functional, psychosocial, and environmental needs
• Implement and revise the individualized plan of care
• Compliance with AdvisaCare Hospice Policies and Procedures and Medicare regulations
• Consult with and educate the patient/family on disease process, self-care techniques, end-of-life care, and ethical concerns
• Supervise paraprofessionals providing services to the patient
• Prepare clinical and progress notes and participate in interdisciplinary group (IDG) conferences
• Participate in the Hospice performance improvement program
• Maintain high customer satisfaction with clients, families, and caregivers
• Communicate information using current processes and technology within the organization
• Conduct supervisory visits to observe and evaluate assigned team members
• Provide feedback during regular interdisciplinary group meetings on the development, review, and revision of the patient/family plan of care

Requirements

• Current Michigan RN license in good standing
• Minimum 1 year of hospice experience
• Empathetic approach to patients and caregivers
• Ability to participate in daily field visits and weekly office meetings as indicated
• Ability to adapt to patient’s status and needs on a full-time basis
