As a Hospice RN Case Manager, you will provide emotional support with the goal to help patients live their final days with dignity and comfort. You will conduct appropriate supervisory visits which allow for clinical observation of performance of assigned team members. Participate and provide feedback to the team for regular interdisciplinary group meetings on the development, review, and revision of the patient/family plan of care.

Responsibilities

  • Initial and ongoing assessment of patient's physical, functional, psychosocial, and environmental needs
  • Implement the individualized plan of care and revisions as needed
  • Ensure compliance with AdvisaCare Hospice Policies and Procedures and Medicare compliance
  • Consult with and educate the patient/family on disease process, self-care techniques, end-of-life care, and dealing with issues of ethical concern
  • Supervise paraprofessionals providing services to patients
  • Prepare clinical and progress notes and participate in IDG conferences
  • Participate in the Hospice performance improvement program
  • Maintain a high level of customer satisfaction with our clients, families, and caregivers
  • Communicate information using current processes and technology within the organization
  • Participate in field visits daily and weekly office meetings as indicated

Requirements

  • Current Michigan RN license in good standing
  • Minimum 1 year of hospice experience
  • Empathetic approach to patients and caregivers
  • Ability to adapt to patient's status and needs on a full-time basis
