Field Care Specialist I (Springfield, MO / Field-based)

About the role

We are seeking a Field Care Specialist I for our Missouri team. This role involves frequent travel throughout Springfield, MO, with occasional visits to our local FreedomCare office. Key responsibilities include managing an assigned patient caseload, adhering to service standards, and conducting home visits to ensure compliance.

Responsibilities

• Manage an assigned patient's caseload
• Ensure compliance with monthly service standards
• Respond to patient and caregiver inquiries and concerns
• Input and review data for case management
• Build rapport and confidence with patients and caregivers
• Multitask and maintain excellent communication skills
• Conduct home visits and document findings
• Work collaboratively with Care Coordinators for scheduling and follow-up

Requirements

• 1+ years of experience in field-based roles
• Ability to assess patient health risks and identify home safety hazards
• Strong motivation to meet case management targets
• Excellent written and verbal communication skills
• Proactive problem-solving and critical thinking abilities
• Self-starter personality and ability to work independently
• Reliable transportation and ability to travel daily

Qualifications

• High school diploma or equivalent
• Valid driver’s license and reliable vehicle
• Basic computer skills, including proficiency with Salesforce and DocuSign

Pay

The base pay for this role ranges from $20.00 to $23.00 per hour at the commencement of your first year of employment. Compensation is determined based on various factors including skill set, experience, education, and location.

This position involves frequent travel throughout the Springfield, MO area, with occasional visits to our local FreedomCare office. Exact schedules are flexible and tailored to each individual's needs.
