About the role
The Family Support Program (FSP) and Specialized Family Support Program (SFSP) provide intensive mental health services and supports to youth with severe emotional disturbances. The FSP aims to strengthen family stability, improve clinical outcomes, and promote community-based services. The SFSP offers crisis stabilization, community mental health, and assessment services for youth at risk of custody relinquishment.
Responsibilities
- Provide clinical assessment, treatment planning, crisis intervention, coordination with HFS and residential entities, collaboration with hospital staff, case management, community support, therapy/counseling, and direct clinical interventions.
- Maintain enrollment and link recipients to community-based and/or residential services.
- Communicate weekly with clients and families to ensure stabilization goals are met.
- Complete and submit required HFS reports.
- Develop and provide diagnostic assessments and treatment plans.
- Engage clients in clinical therapeutic rehabilitative services, case management, community support, and crisis intervention.
- Provide consultation with other professionals.
- Prepare and oversee case presentations for clinical review.
- Ensure clients are linked with appropriate resources.
- Maintain accurate documentation and adhere to HIPAA guidelines.
- Perform other duties as assigned.
Requirements
- Master's degree in counseling, guidance, rehabilitation counseling, social work, psychology, family therapy, or a related field.
- At least 1,000 hours of practicum/internship experience or one year of clinical experience in a mental health setting.
- Valid driver’s license and acceptable motor vehicle report.
Qualifications
- Knowledge of FSP and SFSP criteria and requirements.
- Ability to communicate and coordinate with potential FSP candidates.
- Experience in crisis stabilization, community mental health, and assessment services.
- Skills in clinical assessment, treatment planning, and therapeutic interventions.
- Knowledge of Illinois Department of Human Services, DCS, and IHFS rules and regulations.
- Ability to maintain client confidentiality and adhere to HIPAA guidelines.
