Family Nurse Practitioner / Physician Assistant (NP/PA)

About the Role

Family Nurse Practitioner (FNP) or Physician Assistant (PA) providing care for pediatric and adult patients in a hub-based setting.

Responsibilities

• Conduct comprehensive clinical intake visits, manage chronic conditions, and provide preventative care.
• Follow up proactively with members, ensure accurate diagnosis coding, and address quality gaps.
• Prescribe medications for mental health and substance use disorders, and support members' care goals.
• Refer to specialists as needed, stay current on evidence-based primary care practices, and collaborate with interdisciplinary teams.
• Participate in case conferences and contribute to care plan development.
• Provide direct primary and wrap-around care, support chronic disease management, behavioral health, and wellness initiatives.
• Work collaboratively within a multidisciplinary team to optimize patient outcomes.

Requirements

• 3+ years of patient-facing experience.
• Completion of accredited NP or PA program with national certification.
• Unrestricted state license, board certification or eligibility, and BLS certification.
• Willingness to obtain DEA licensure.
