Family Nurse Practitioner / Physician Assistant (NP/PA)

Role: Family Nurse Practitioner (FNP) or Physician Assistant (PA) providing care for pediatric and adult patients in a hub-based setting. Responsibilities Conduct comprehensive clinical intake visits, manage chronic conditions, and provide preventative care.Follow up proactively with members, ensure accurate diagnosis coding, and address quality gaps.Prescribe medications for mental health and substance use disorders, and support members' care goals.Refer to specialists as needed, stay current on evidence-based primary care practices, and collaborate with interdisciplinary teams.Participate in case conferences and contribute to care plan development. Qualifications & Skills 3+ years of patient-facing experience.Completion of accredited NP or PA program with national certification.Unrestricted state license, board certification or eligibility, and BLS certification.Willingness to obtain DEA licensure. Key Duties: Provide direct primary and wrap-around care, support chronic disease management, behavioral health, and wellness initiatives, and work collaboratively within a multidisciplinary team to optimize patient outcomes.
