Key Responsibilities

• Offer room and board to one or two individuals with disabilities.
• Provide person-centered direct personal care, including activities of daily living such as cooking, bathing, cleaning, medication administration assistance, and accompanying individuals to medical appointments.
• Support individuals in achieving independence and working towards their personal goals and outcomes outlined in their PCSP (Person-Centered Support Plan).

Qualifications and Experience

• Minimum of 1 year of experience working with individuals with intellectual disabilities.
• Criminal background check is a must.
• Access to a safe and accessible home or apartment with a dedicated bedroom for the individuals, especially if wheelchair accessibility is required.
• Reliable vehicle and valid auto insurance and driver's license for transportation.
• Completion of all required training, including Relias, Medication Administration Certification, CPR and First Aid.
• Personal computer with internet connectivity for documentation purposes.
• High school graduate or equivalent education preferred.
