About the role
Benchmark Human Services is seeking a Qualified Intellectual Disabilities Professional (QIDP) to join our team in Fort Wayne, IN. Since 1960, we have been dedicated to empowering individuals with intellectual and developmental disabilities to live independently and be included in their communities.
Responsibilities
- Provide case management for residents in group homes, administering assessments and overseeing resident program plans.
- Develop and monitor the implementation of client's individual plans.
- Complete daily, weekly, monthly, and yearly documentation as required by Benchmark Human Services and state/federal regulations.
- Maintain client and group home files to ensure all documentation is current.
- Plan and participate in recreational, therapeutic, and training activities for clients.
- Seek input from the IDT and supervisor, and maintain effective communication.
- Safeguard client rights and coordinate with the designated Human Rights Committee when necessary.
- Provide training and supervision to staff in accordance with state and federal regulations and Benchmark Human Services policies.
- Monitor overall health care needs, including plans, medications, treatment protocols, and the general well-being of clients.
- Attend agency and departmental meetings, client appointments, and chair client-specific and house meetings.
- Provide on-call services and respond to client and house needs.
- Report any suspected abuse, neglect, or exploitation to supervisor or department head.
- Report reasonable suspicion of a crime against an ICF/MR resident to Indiana State Department of Health and Law Enforcement (Elder Justice Act).
- Perform other duties as assigned.
Qualifications
- Bachelor’s degree in human services field.
- One (1) year experience working with people with developmental disabilities, preferably in a residential setting.
