About the role
Clinton Manor Living Center is home to about 51 individuals with developmental disabilities who have high medical needs. We believe in promoting independence and advocating for individuals to live with integrity, dignity, and freedom of choice, enhancing their quality of life and ensuring their rights are upheld.
The QIDP's primary purpose is to ensure individuals receive Person-Centered Planning and Care, providing the individualized services needed to empower them to make choices about their services, enhance community inclusion, and encourage them to live as they choose with integrity, dignity, and quality of life. This position manages a caseload of approximately 10-12 residents and supervises a team of Direct Support Professionals who provide direct support.
Schedule
Mainly Monday–Friday, 8 am–4 pm or 9 am–5 pm, including one day a week on an 11 am–7 pm shift. Flexibility is required to cover DSP shifts as needed, including rotation of on-call responsibilities.
Pay
Hourly wage of $29–$33 based on experience, plus a stipend for on-call responsibility.
Responsibilities
- Provide Person-Centered Planning and Care for individuals with developmental disabilities.
- Empower residents to make choices about their services and enhance community inclusion.
- Manage a caseload of approximately 10-12 residents.
- Supervise a team of Direct Support Professionals.
- Cover DSP shifts and on-call responsibilities as needed.
Requirements
- Must pass a criminal background check based on Healthcare Worker Registry regulations.
- Must pass various State and Federal registry checks.
- Must meet requirements of a pre-hire physical.
- At least one year of experience working directly with individuals with developmental disabilities.
- Bachelor’s degree in Social Work, Sociology, Special Education, Psychology, or a closely related human services field, or be a licensed Registered Nurse.
- Valid driver’s license and ability to meet driving requirements.
