Epic Healthy Planet / Population Health Analyst Remote 6-12+Months Build, optimize, and support Epic Healthy Planet for clinical and analytics workflows. Develop and maintain population health registries; optimize registry performance. Create, maintain, and optimize Reporting Workbench reports. Design and support workflows for care gap management and closure. Facilitate quality reporting, including HEDIS and eCQM measures. Develop and maintain care management workflows, including care plans and assessments. Collaborate cross-functionally with clinical, quality, and operational leaders. Support population health and value-based care initiatives. Participate in Compass Rose program implementation and build (preferred). Manage patient attribution, roster management, and assignment workflows (preferred). Integrate external claims data and social determinants of health (SDOH). Utilize risk stratification and predictive analytics tools. Develop and validate reports using Clarity and Caboodle. Support implementation of Medicare Stars and other value-based programs. Build and maintain population health dashboards and analytics. Lead bulk outreach and patient engagement campaigns. Validate data across Epic, Clarity, Caboodle, and downstream reporting platforms. Conduct upgrade testing, optimization, and provide end-user support.
