REMOTE WORK Epic Healthy Planet Analyst ____w2

Requires Epic Healthy Planet certification. Prefer experience with Compass Rose/advanced risk stratification tools. Must have experience integrating external data sources (claims, SDOH, HIE feeds) into Healthy Planet. Strong knowledge of quality measure logic (HEDIS, eCQMs). Familiarity with care management documentation and utilization workflows. Day-to-Day Responsibilities Configure, optimize, and support Healthy Planet build (registries, risk models, care gap logic, patient attribution). Analyze population health workflows and translate needs into system design/enhancements. Develop and maintain registries, reports, dashboards, and analytics for value-based care/quality programs. Collaborate with Population Health leadership and clinical teams to identify care gaps and implement interventions. Design, build, and maintain care management tools (care plans, protocols, assessments, outreach workflows). Support payer and value-based contract requirements (accurate data capture, attribution, measure calculation). Analyze/troubleshoot data integrity issues for population health metrics and reporting. Work with data/analytics teams to validate data flows between Epic, Clarity/Caboodle, and reporting platforms. Lead/support workflow optimization, automation, and reporting enhancements. Participate in governance/prioritization for Population Health enhancements (align with goals/regulations). Support testing, validation, and change management for Healthy Planet upgrades/releases. Provide end-user support and issue resolution for Population Health stakeholders. Assist onboarding of new programs/service lines into Healthy Planet (registry design, measure mapping, workflow alignment). Maintain documentation for build standards, workflows, and reporting logic.
