Requirements
- Epic Healthy Planet certification (required)
- Experience with Compass Rose and advanced risk stratification tools
- Experience integrating external data sources — claims, SDOH, HIE feeds — into Healthy Planet models
- Strong background in quality measure logic (HEDIS, eCQMs)
- Familiarity with care management documentation and utilization workflows
