Enhanced Service Coordinator
About the role
The Service Coordinator at The Village of Spring Meadows, Jackson, Michigan, plays a crucial role in coordinating programs and services to help residents maintain a good quality of life and age in place. This position involves assessing and identifying resident and family needs, networking with community organizations, and providing various forms of support.
Responsibilities
- Assess and identify resident and family needs and outside resources to meet those needs.
- Initiate and complete psychosocial assessments, interviewing patients, families, and others to evaluate current conditions and identify adjustments.
- Provide general case management and referral services, maintaining complete and updated resident files.
- Monitor and manage supportive services as needed.
- Carefully coordinate services for optimal living, including aging in place and wellness philosophies.
- Complete Care Plans/Quarterly Monitoring for residents needing referrals and follow up on all services post-implementation.
- Submit Semi-Annual Performance Reports to HUD to maintain funding for the Service Coordination program at the property.
- Engage all residents to identify areas of need and make necessary referrals.
- Organize and promote resident involvement in on and off-site programs.
- Aid in the coordination of home care services to enable independent living and aging in place, including developing a Resource Directory listing service providers.
- Continuously educate residents (and staff) on various matters including service availability, application procedures, resident rights, health/safety/fitness, and issues related to aging.
- Coordinate counseling, occasional visits from a nurse, preventative health screenings/wellness, and legal advocacy.
Requirements
A minimum associate's degree or equivalent in social work, psychology, or gerontology is preferred. Two or more years of experience in social service delivery with the elderly and/or family population is a plus. Demonstrated working knowledge of supportive services and other resources in the area served by the project, along with demonstrated ability to advocate, organize, problem-solve, and provide results for the residents served, are also required.
Qualifications
- Good communication, comprehension, and interpersonal skills.
- Working knowledge of Windows, Microsoft Office Suite, and resident software data systems (AASC online, Care Guide, RealPage, OneSite, or similar related systems).
