About the role
The Qualified Mental Health Professional's primary focus is face-to-face mental health assessment to determine the need for treatment in a hospital setting. They must engage in assessment, safety, and discharge planning.
Responsibilities
- Engage in assessment, safety, and discharge planning
- Develop, conduct assessments, individual service plans, and treatment plans
- Document accurately and complete all documents
- Maintain records and data entry
- Coordinate appropriate levels of care
- Obtain collateral information
- Determine inpatient, crisis stabilization, outpatient, and mobile services
- Determine and implement discharge plans, secure referral appointments, and facilitate all necessary lines of communication
Requirements
- Master’s degree in social work, marriage and family therapy, professional counseling, psychology, or a 4-year Registered Nurse degree certified in psychology with 2 years of mental health experience
- Kentucky licensure to practice as LCSW, LPCC, Psychology Practitioner, Licensed Psychology Associate, or certified psychologist
- Two to three years’ experience providing case management to individuals with mental health illness or addiction
- Demonstrated experience developing, conducting assessments, individual service plans, treatment plans, and documentation abilities
