Qualifications & Skills

• Minimum Education: Nursing Diploma/Associate's Degree
• Licensure: RN
• No prior experience required; preferred candidates may have a Bachelor's Degree in Nursing and less than 2 years of experience

Key Duties

• Assess and evaluate patient needs across physical, emotional, and psychosocial domains
• Educate patients and families on managing health conditions and post-treatment care
• Collaborate with the interdisciplinary team to create and communicate care plans
• Support safety, quality, and compliance initiatives

Develops and Documents Individualized Care Plans

Develops and documents individualized care plans collaboratively with the care team, considering patient preferences and barriers.

Evaluates Physical, Psychosocial, Emotional, Cultural, and Spiritual Needs
