Job Summary

The Director of Population Health at La Clinica de Familia (LCDF) plays a crucial role in driving strategic oversight of population health programs. This includes Chronic Care Management, Certified Community Behavioral Health Clinic (CCBHC) Care Coordination, and Community Health Worker (CHW) initiatives. The position aims to improve health equity, quality, and cost through high-quality, patient-centered care, value-based performance, and regulatory compliance.

Job Requirements

• Bachelor’s degree in Nursing; Master’s degree preferred
• A minimum of 2 years of Clinical Health outcome experience preferred
• Previous management experience preferred
