The Director, CHW Operations is responsible for building and scaling a high-performing Community Health Worker (CHW) function within a growing healthcare organization. Key responsibilities include:
- Overseeing the strategy, execution, and continuous improvement of community health operations.
- Ensuring CHW teams deliver consistent, high-quality support across markets.
- Managing workforce planning, deployment strategies, productivity management, and performance optimization.
- Building management frameworks that promote coaching, accountability, quality improvement, and professional development.
- Partnering with clinical and care delivery teams to integrate CHWs into multidisciplinary care models.
- Monitoring operational metrics and identifying opportunities to improve member engagement, retention, care coordination, and health outcomes.
- Developing and mentoring frontline leaders, fostering a culture of ownership, collaboration, and continuous improvement.
- Driving process improvements to enhance the effectiveness and scalability of community-based healthcare programs.
Requirements
- Experienced healthcare operations leader with a strong background in managing community-based healthcare teams and improving care delivery outcomes.
- Proven experience managing Community Health Workers, care coordinators, patient navigators, outreach teams, or similar community-based healthcare workforces.
- Experience building and leading distributed field teams across multiple locations or markets.
- Strong understanding of Medicaid populations, social determinants of health, and barriers impacting healthcare access.
- Demonstrated success developing frontline leaders and improving operational performance through structured processes and accountability systems.
- Excellent communication, coaching, stakeholder management, and change leadership skills.
- Experience with programs such as ECM, Community Supports, CalAIM, or other Medicaid care management initiatives is preferred.
- Background working with culturally diverse and historically underserved communities is highly valued.
- Experience scaling healthcare operations during periods of rapid growth is a plus.
- Bilingual Spanish/English proficiency is strongly preferred.
