Director, Case Management | Acute Rehab

About the role

We are seeking an experienced Director of Case Management to provide strategic and operational leadership for case management services supporting our acute rehabilitation programs at Inova Mount Vernon Hospital and Inova Center for Personalized Health in Fairfax. This leader will oversee case management operations, drive effective care coordination and transitions, and partner with interdisciplinary teams to optimize patient outcomes, access, and the overall patient experience. The Director of Case Management leads and coordinates key strategic development and functional services to support department objectives and organizational goals. Maintains effective and responsive relationships with patients, attending medical staff and other departments. Manages department finances, including budgeting and control of salary, non‑salary and capital investment expenses. Manages professional staff including day‑to‑day supervision, team member relations and staff development.

Responsibilities

• Leads all social work and discharge planning activities within assigned operating unit.
• Oversees the day‑to‑day operation of all Case Management related activities, including the preparation, distribution and interpretation of monthly reports on productivity and Case Management targets.
• Leads the management, measurement and continuous quality improvement of length of stay, avoidable days and denial management.
• Oversees the maintenance of the Case Management database and use of Case Management software.
• Responds promptly to requests from other departments within the operating unit and maintains cooperative relationships with corresponding departments at other operating units.
• Ensures adherence to service excellence standards and the provision of outstanding service to internal and external customers.
• Manages reimbursement and contract issues through collaboration with managed‑care contracting.
• Selects, hires and trains staff to perform assigned functions at a competent level, including Case Managers and administrative staff.

Minimum Requirements

• Five years of experience in case management or acute care, including at least one year of management experience.
• Bachelor’s Degree.

Preferred Requirements

• CCM/ACMA certification.
• Two years of progressive leadership experience in case management or care coordination within an acute rehabilitation care setting.
