About the role
Lead an on-site case management team at a high-census 50-bed inpatient rehabilitation hospital in Parkersburg, WV. The hospital maintains an average daily census of 49 with typical lengths of stay of 7–14 days. Case Management reports directly to the hospital CEO and receives additional support from a Regional Director of Case Management.
Responsibilities
- Direct day-to-day operations and human resource management of the Case Management department
- Lead a team of 4 full-time Case Managers, 1 full-time Case Management Assistant, and 2 PRN Case Managers (mix of RNs and social workers)
- Oversee census management, patient care outcomes, utilization review, and the discharge planning process
- Ensure the interdisciplinary plan of care is effective, appropriate, and aligned with patient and family needs
- Serve as a patient and family advocate while managing appropriate utilization of resources
- Support the team by covering patient caseloads when needed (typical CM caseload is 12–15 patients; the director does not usually carry a standing caseload)
- Partner with hospital leadership and the Regional Director of Case Management on department performance and key care indicators
- Maintain compliance with hospital, state, and payer requirements related to case management, UR, and discharge planning
Qualifications
- Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, or Rehabilitation Counseling)
- If licensure is available for the discipline in West Virginia, must hold an active license
- For Nursing: bachelor’s degree in nursing (BSN) with RN licensure
- For other eligible health care professionals: minimum of a bachelor’s degree; graduate degree preferred
- Open to social work degrees, licensed clinical social workers, and master’s degrees in a healthcare-related field
- 3 years of hospital-based Case Management experience including Utilization Review and Discharge Planning
- Demonstrated on-site leadership experience leading case managers in a healthcare / hospital setting
- Proven longevity and stability in prior roles
- Clinical licensure preferred but not required
- CCM or ACM required within the first year if not currently certified (employer will assist)
