About the role

We are opening a brand new rehab hospital in Winter 2026 and are seeking an experienced Director of Case Management (RN) to oversee the hospital-wide case management program. This role involves coordinating care, ensuring proper utilization of hospital services, and facilitating timely discharge planning. You will supervise case managers, promote quality of care through interdisciplinary collaboration, and ensure regulatory compliance and fiscal planning.

Responsibilities

  • Oversee the review of patient medical records to ensure proper utilization of hospital services.
  • Assist case managers in providing timely coordination of care and discharge planning.
  • Ensure screening of all admissions to determine the appropriate level of care.
  • Supervise case managers and daily operations of the case management department to facilitate optimal financial and clinical outcomes.
  • Handle fiscal planning and ensure regulatory compliance.
  • Analyze current systems and variances to identify opportunities for improvement.
  • Collaborate with the interdisciplinary team to promote quality of care.

Requirements

  • RN licensure in the state where the hospital resides (required). Social workers, MSW, LSW, PT, OT, SLP, and RT licensure may also be accepted in accordance with state regulations.
  • Current BLS certification (required).
  • BSN/MSN (preferred).
  • Certification in an approved Case Management Program (preferred).
  • Three to five years of case management experience (required).
  • Prior experience in IRF or LTACH (preferred).
  • Prior management experience (preferred).
