The Director of Case Management
- Oversees review of patient’s medical record to ensure proper utilization of hospital services.
- Aids case managers in providing timely coordination of care and discharge planning.
- Screens all admissions to determine the appropriate level of care.
- Supervises case managers, overseeing the daily operations of the case management department to facilitate optimal financial and clinical outcomes.
- Analyzes current systems and variance to identify opportunities for improvement and works to promote quality of care through collaboration with members of the interdisciplinary team.
Education and Training
- RN licensure in the state where the hospital resides required.
- Social workers may also be accepted in lieu of RN licensure, if they are licensed or hold a bachelor's degree in social work, as required and in accordance with state regulations.
- MSW, LSW, PT, OT, SLP and RT licensure may be accepted in accordance with state regulations.
- Current BLS certification required.
- BSN/MSN preferred.
- Certification in an approved Case Management Program preferred.
Experience
- Three to five years of case management experience required.
- Prior experience in IRF or LTACH preferred.
- Prior management experience preferred.
