We seek a Direct Support Professional - 24 HC who embodies our mission and joins us in making a positive impact in the community.

What you will be doing?

• Implement Individualized Strategies under the direction of a Qualified Intellectual Disabilities Professional.
• Document service provided including recreation, leisure, and activities of daily living.
• Maintain client rights, dignity, and privacy.
• Ensure a clean and safe environment.
• Work with clients experiencing maladaptive behaviors and require intensive care.
• Assist with meal preparation, toileting, bathing, and grooming.
• Obtain and administer medications safely.
• Maintain cleanliness and safety in assigned buildings and vehicles.
• Monitor and report emergency situations.
• Provide transportation for clients.
• Communicate with healthcare providers.
• Participate in community and in-house activities.
• Conduct client input meetings and other meetings as assigned.

Essential Job Responsibilities

• Maintain a team approach.
• Implement specific programs from the Individual Plan or Implementation Strategy.
• Familiarize and adhere to all agency and program rules and regulations.
• Report concerns to supervisor promptly.
• Plan and prepare meals.
• Tend to self-care needs.
• Obtain medication administration authorization.
• Maintain medication administration authorization.
• Ensure safe conditions inside and outside the home.
• Perform daily housekeeping tasks.
• Monitor and relay medical appointment information.
• Attend meetings, shifts, and trainings.
• Assist in the orientation/training of new staff.
• Participate in community and in-house activities.
• Take verbal and written directions.
• May be responsible for independent building coverage.

Qualifications

• High School Diploma or GED.
• CNA (Certified Nursing Assistant) or Autism Certificate desired.
• Experience working with developmentally disabled adults preferred.
• Physical requirements: climb stairs daily, push/pull/reach/stoop/bend over, use step stool daily, perform daily housekeeping tasks, assist clients in independent living skills, physically assist clients, implement behavior support strategies, respond to emergency behavioral issues, significant walking and standing daily, lift up to 50 pounds, pass S-TOFHLA literacy test, complete DSP and Medication Administration Training, maintain medication administration authorization.
• Equipment: computer and software, household appliances, alarm systems, telephone, office equipment, adaptive equipment, agency vehicles, personal automobile, cell phone, and access to IDOT Approved Driver Status.
• Additional requirements: at least 18 years of age, valid Illinois driver's license, access to a personal cell phone with secure messaging capabilities, meet and maintain IDOT Approved Driver Status.

Physical Requirements

• No medical restrictions that would prohibit implementation of a lift/transfer or behavioral programs.
• Perform daily housekeeping tasks and assist clients in independent living skills.
• Physically assist clients in situations including, but not limited to: getting in and out of van, bathtub, shower, or bed regularly.
• Implement behavior support strategies and respond to emergency behavioral issues.
• Requires significant walking and standing on a daily basis.

Additional Requirements

• Must be able to lift or move up to 50 pounds to assist in two-person lift or with use of adaptive equipment.
• Pass S-TOFHLA literacy test.
• Complete required DSP and Medication Administration Training, including, CPR, First Aid, Infection Control, OIG Rule 50, and CPI.
• Maintain medication administration authorization by following all policies and procedures.
• Evening, weekend, overnight, and holiday hours required.
