Digital Health Specialist - Hybrid

About the Role

This is a full-time, hybrid position based in Jacksonville, FL, combining in-clinic and remote patient care.

Responsibilities

• Provide outpatient physical therapy services for diverse orthopedic and neurologic conditions.
• Develop personalized care plans and measure patient outcomes using advanced software.
• Deliver telehealth evaluations, assessments, and treatments via digital platforms.
• Perform Remote Therapeutic Monitoring (RTM) and virtual care, supporting digital health program growth.
• Collaborate with clinicians and support operational needs for digital services.
• Communicate with providers to update treatment plans and serve as a liaison with the Digital Health team.
• Analyze service metrics and participate in program development and marketing efforts.

Requirements

• Diploma from a CAPTE-accredited Physical Therapy Program and active state licensure in Florida.
• Minimum of 2 years outpatient clinical experience; digital health experience preferred.
• Commitment to high-quality patient care and proficiency with digital health tools.
