Digital Health Specialist - Hybrid

We are seeking a Physical Therapist with digital health expertise for a hybrid role combining in-clinic and remote services. Responsibilities Include Providing personalized outpatient physical therapy care for diverse orthopedic and neurologic patientsDeveloping and implementing treatment plans, documenting evaluations, treatments, and outcomesDelivering telehealth services, including evaluations, assessments, and remote therapeutic monitoringUsing digital platforms for virtual care and monitoring patient progressCollaborating with clinicians and supporting digital health program growthServing as a liaison between patients, providers, and digital health leadershipAnalyzing regional service data and participating in program development and marketing Qualifications And Skills Degree from a CAPTE-accredited PT programFlorida PT licensure or in process, with willingness to obtain licensure in other statesMinimum of 2 years outpatient clinical experienceExperience with digital health services preferredStrong commitment to quality patient care and effective communication Additional Information This is a full-time, hybrid position based in Jacksonville, FLInvolves developing digital health programs and providing in-person and virtual therapyOffers competitive pay, benefits, professional development, and growth opportunities
