Digital Health Specialist - Hybrid

About the Role

This is a full-time, hybrid position based in Jacksonville, FL, combining in-clinic and remote services to deliver personalized outpatient physical therapy and support the growth of digital health programs.

Responsibilities

• Provide personalized outpatient physical therapy care for diverse orthopedic and neurologic patients
• Develop and implement treatment plans, documenting evaluations, treatments, and outcomes
• Deliver telehealth services, including evaluations, assessments, and remote therapeutic monitoring
• Use digital platforms for virtual care and monitor patient progress
• Collaborate with clinicians and support digital health program growth
• Serve as a liaison between patients, providers, and digital health leadership
• Analyze regional service data and participate in program development and marketing

Requirements

• Degree from a CAPTE-accredited physical therapy program
• Florida PT licensure or in process, with willingness to obtain licensure in other states
• Minimum of 2 years outpatient clinical experience
• Experience with digital health services preferred
