Digital Health Specialist - Hybrid

About the role

This full-time position combines in-clinic and remote work, requiring the candidate to work three days in the Jacksonville, FL clinic and two days remotely as a Digital Health Specialist.

Responsibilities

• Providing personalized physical therapy care for patients with orthopedic and neurologic conditions.
• Developing and implementing treatment plans, measuring patient outcomes, and documenting evaluations and progress.
• Delivering telehealth services, including evaluations, assessments, and remote therapeutic monitoring (RTM).
• Utilizing digital platforms for virtual care, monitoring, and communication with patients and providers.
• Serving as a liaison between patients, in-clinic providers, and the Digital Health team to optimize remote services.
• Supporting program development, data analysis, and marketing efforts related to digital health initiatives.

Qualifications & Skills

• Diploma from a CAPTE-approved Physical Therapy Program and FL state license (or in process).
• Minimum of 2 years outpatient clinical experience; digital health experience preferred.
• Strong commitment to quality patient care and effective communication skills.

Key Duties

• Delivering in-person and telehealth physical therapy services.
• Supporting the growth and development of digital health programs.
• Monitoring patient progress and adapting treatment plans accordingly.
• Collaborating with healthcare providers and the Digital Health team to enhance remote care delivery.
