Responsibilities

• Perform comprehensive nutrition screenings and assessments.
• Develop and implement individualized Medical Nutrition Therapy (MNT) plans.
• Document nutrition assessments, interventions, and outcomes within the electronic medical record.
• Provide nutrition groups, education and counseling to patients, families, and caregivers.
• Participate in facility-wide and departmental Quality Assurance and Performance Improvement initiatives.
• Participate in multidisciplinary patient care rounds and departmental meetings.
• Support staff development initiatives and contribute to improved clinical outcomes.
• Complete Swallowing Screen as needed.

Requirements

• Bachelor's or Master's Degree in Clinical Nutrition, Dietetics or a related field from an accredited institution AND a Registered Dietician (RD) or Registered Dietitian Nutritionist Credential.

Background Check

The agency provides their background check, however we also require the candidate to complete the SCR Form which we run and we run the SEL-staff exclusion list in tandem with the agency's background check.

Vaccination Requirements

• Immunization Records (MMR) – (Titers if MMR's are older than 10 years)
• PPD (within the past year)
• Proof of immunizations must include MMR and TB via PPD test.
• A flu vaccine is preferred.
• Please send immunization documents in a single pdf file.

• HPC Orientation Training
• TRUST Initial Training
• BLS/First Aid

Schedule

Monday - Friday 8:00AM - 4:30 PM, up to 2 years but not guaranteed.
