Dietician Manager (Cantonese/Mandarin)

Job Summary

The PACE Dietitian Manager is responsible for the medical nutrition program for PACE participants. They participate as a PACE Interdisciplinary Team (IDT) member and complete nutrition assessments for new and existing PACE participants. They develop and implement nutrition and diet care plans, monitor participants for changes in health or psychosocial status, and provide nutrition and diet education.

Essential Job Functions

• Participates as a PACE Interdisciplinary Team (IDT) member and completes nutrition assessments for new and existing PACE participants.
• Collaborates with the primary care provider and IDT to develop and implement nutrition and diet care plans, addressing medical, physical, social, and emotional needs.
• Affirms PACE participants' food security status and recommends interventions to the IDT.
• Affirms PACE participants' nutritional supplement needs and monitors for changes in health or psychosocial status requiring a nutrition reassessment.
• Presents pertinent changes in participant nutritional or dietary status to the IDT and makes recommendations for incorporating or modifying therapeutic diets and dietary modifications to participant care plans.
• Plans food services at the PACE Center, ensuring dietary restrictions are enforced and participant food preferences are considered.
• Supervises, mentors, and evaluates nutrition staff; oversees staffing, scheduling, and performance to ensure continuity and accountability.
• Completes and maintains documentation in the PACE EHR in accordance with federal PACE regulations and organizational policies.
• Serves as a nutrition and diet subject matter expert and communicates best practices in nutrition management for a frail elderly population.
• Provides nutrition education to participants and caregivers, including conducting home visits if necessary.
• Assists with meal and snack service in the PACE Center as needed.
• Supports the PACE Quality Improvement (QI) program and participates in QI activities as assigned.

Minimum Requirements

• Bachelor’s degree in Nutrition from an ADA-accredited institution
• Certified as a Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN)
• Minimum 1-year work experience as a Dietitian
• Registered member of the ADA and registered by the Commission on Dietetic Registration
• Leadership and management experience
• Bilingual ability (Cantonese and/or Mandarin) and experience providing culturally and linguistically appropriate services
• One year of experience working with a frail or elderly population, or appropriate training upon hiring
• Meet a standardized set of competencies for the Dietitian position established by the PACE plan
• Experience working in Electronic Health Records (EHRs)
• Be medically cleared for communicable diseases and have up-to-date immunizations and vaccines
• Not be excluded from participating in Medicare and Medicaid programs
• Not be convicted of criminal offenses pertaining to physical, sexual, drug, or alcohol abuse
• BLS certified
• ServSafe certified
