Description And Responsibilities

Open Sky's Day Habilitation programs serve adults with intellectual disabilities, often alongside mental health diagnoses and/or physical challenges. Participants typically require supportive structures and staff assistance to engage in program activities and community involvement. Our staff educate through individualized service plans, daily curriculum, and emphasize community integration.

• Provide direct care support to individuals served, ensuring a supportive and structured environment.
• Guide, advocate for, and teach life skills to individuals within the Day Program.
• Implement and review all service and behavior plans with support from the interdisciplinary team.
• Complete required documentation to ensure the health, safety, and well-being of all individuals.
• Facilitate the development of meaningful community roles and personal relationships.
• Participate in team meetings, supervision sessions, and other assigned meetings.

Qualifications

• A High School Diploma, GED, or equivalent, required.
• A valid Drivers License and acceptable driving record, required.
• Drug testing required.
• Reliability, willingness to learn, and openness to new opportunities.
