About the role

Responsibilities

• Provide support to individuals in a direct care role, ensuring a supportive and structured environment.
• Guide, advocate, and teach life skills to individuals served within the Day Program.
• Implement and review all service and behavior plans with support from the interdisciplinary team.
• Complete required documentation to ensure the health, safety, and well-being of all individuals.
• Facilitate the development of valued community roles and personal relationships.
• Attend and participate in team meetings, supervisions, and other assigned meetings.

Requirements

• A High School Diploma, GED, or equivalent is required.
• A valid drivers license and acceptable driving record are required.
• A reliable transportation method is necessary.
