Crisis Care Manager - Casa Grande
About the role
Horizon Health and Wellness, Inc. is seeking a Crisis Care Manager to support individuals experiencing behavioral health crises in the Pinal County community. The position involves providing face-to-face crisis assessment and intervention, managing an outpatient caseload, and offering mobile crisis services.
Responsibilities
- Provide crisis assessment and intervention to individuals from all age groups experiencing behavioral health crises.
- Intervene to alleviate immediate social, emotional, or environmental crises.
- Assess, evaluate, triage, and refer participants for appropriate follow-up treatment.
- Co-manage an outpatient caseload for low-need individuals, including case management, care coordination, assessment, and treatment planning.
- Work within the Apache Junction and Casa Grande locations, operating a Wednesday shift from 7 am - 5 pm and Thursday-Saturday shifts from 5 am - 3 pm.
Requirements
- Required: High School Diploma/GED with 4 years of behavioral health experience OR Bachelor’s degree in a behavioral health-related field.
- Experience equivalency considered if combined education and experience meet qualifications as a Behavioral Health Technician (BHT).
- Must be at least 21 years old and have a valid driver’s license with an acceptable driving record.
- Trained in crisis intervention and capable of providing services to agency participants and residents in the community.
- Must be able to obtain and maintain an Arizona Level One Fingerprint Clearance Card and pass a background check with Pinal County Sheriff's department.
- Must be certified in First Aid and CPR, and agency will provide training if candidate does not have active certification.
- Must demonstrate knowledge of special populations such as substance abuse, and the ability to develop treatment plans based on participants' needs.
- Must have long periods of sitting, the ability to lift up to 50 pounds, and operate various office equipment.
Qualifications
- Minimum of High School Diploma/GED with four (4) years of behavioral health experience OR bachelor’s degree in behavioral health related field.
- At least 21 years of age.
- Valid driver’s license with acceptable driving record.
- Trained in crisis intervention and capable of providing services to agency participants and residents in the community.
- Knowledge of special populations such as substance abuse, and the ability to develop treatment plans based on participants' needs.
- Knowledge of referral resources available to participants.
- Able to prioritize workload, make effective decisions under pressure, and handle conflict.
- Excellent verbal communication skills, analytical and problem-solving skills, and the ability to think on one's feet.
- Physical abilities to sit for long periods, lift up to 50 pounds, and operate various office equipment.
